Preliminary Information
An important part of our preparation for the first session with our clients is to have a completed Intake Form before meeting. This gives us an opportunity to discern specific areas of need for the client that may be beneficial as a starting point.
The Intake Forms are specifically designed by category:
> Marriage Counseling
> Pre-Marital Counseling
> Teen Counseling (Male & Female)

Disclosure Statement and Agreement forms are included for each Intake Form. This form must be signed, dated and returned with the individuals completed Intake Form before counseling begins.
